As I have said in my previous solutions, make sure the difficulty is turned right down to make it easier for you and remember to save regularly.

After lighting the flame, speak with Sheogorath and he will tell you about becoming a duke/duchess of a realm. Go and speak to the two other men in the Sacellum and they will tell you how to become leader of either Mania or Dementia. You need to speak to both of them before speaking with Sheogorath again.

However, before you provide Sheogorath with a decision, make sure to save so that you can reload the save after completing the quest as you can only choose one. I found the best thing to do was save over your old game and then when the decision had been made, create a new save so that when you next saved, that new save would be the one you overwrote and not the save before the decision. I managed to make the mistake of overwriting my save and had to go back a long way.

Speak with Sheogorath and he will tell you that you need to kill the Duke Thadon by poisoning him. When dead, you must take a blood sample. First you need to speak with Wide-Eye who will tell you about Greenmote (the poison), max out her disposition and she will tell you about her routine. At noon, she will make her way to a secret entrance which is where the Greenmote silo is located. Inside, there will be guards-a-plenty that try to arrest you and Wide-Eye in the Silo room itself. Pretty much as soon as I entered, I was spotted so just decided to hack and slash the rest of the way. I would advise trying to get through it without being spotted though as it makes it a lot easier for you later on. When you get to the room, head into the pile and hit A twice to get two lots of the poison. You must then make your way back out via the way you entered. Now that you are out (I was being hounded by guards still at this point), head to the Duke's quarters, the marker on the map will point you there, and pick the lock to get inside. You can use the key if you have it but you would need to have pickpocketed it from Wide-Eye in the silo. I saved before trying and then just hit auto-attempt until it unlocked. You can do it properly though, it's not that hard. When you get inside the quarters, head down the steps and you will find the kitchen to the left right at the end of the corridor. There are two guards in here so you will need to sneak again to avoid being spotted. I was still being chased at this point so just ran through and located the food on the table, it says activate poison rather than steal item and the wine on the middle shelf of a wine rack close to the right-hand table. I, then, let myself get arrested to stop them from chasing me round like madmen. Hopefully though, you will have been sneaking and managed to avoid such a situation. Now that the items have been poisoned, you need to wait until 8pm which is when the Duke will be having his dinner. Enter the room at that time and you will see the Duke die, take the blood sample from his body and head back to the Sacellum Arden-Sul where you can finish the ritual. Place the blood sample in the altar and then talk to Sheogorath. The Duchess of Dementia will come in shortly and a rather lengthy conversation will take place. After this is over, Sheogorath will talk to you again and the achievement will unlock.

*Spoiler!*
This is a quick way to get around some of the sneaking in the misson to become Duke of Mania.
If you either don't have great sneaking ability or can't be bothered with all the sneaking around.

When it comes to poisoning Thadon with the Greenmote you collected on that annoying mission following Wide-Eye. Go to the Halcyon Conservatory. Go to the far end of the Conservatory, there you will find a wooden door to the Duke's Quarters. You will have to lockpick the door. Instead of sneaking past the Golden Saint Guards, just kill them. Get arrested and pay the bounty. When you go back to his quarters (you might have to lockpick the door again) all the guards will be dead and aslong as the cook isn't there you should be free to tamper with the food.
The mission won't work if the cook is in the quarters.
