As I have said in my previous solutions, make sure the difficulty is turned right down to make it easier for you and remember to save regularly.

After lighting the flame, speak with Sheogorath and he will tell you about becoming a duke/duchess of a realm. Go and speak to the two other men in the Sacellum and they will tell you how to become leader of either Mania or Dementia. You need to speak to both of them before speaking with Sheogorath again.

However, before you provide Sheogorath with a decision, make sure to save so that you can reload the save after completing the quest as you can only choose one. I found the best thing to do was save over your old game and then when the decision had been made, create a new save so that when you next saved, that new save would be the one you overwrote and not the save before the decision. I managed to make the mistake of overwriting my save and had to go back a long way.

Now, onto the quest. The achievement unlocks upon completion of the Ritual of Dementia quest.

For this, you have to kill Syl and take her heart. First of all, you need to find Syl's aides Kithlan and Anya Herrick. Speak to them about replacing Syl. If they won't tell you anything, then bribe them to increase their disposition and ask again. Once you have spoken to both, the marker should lead to Syl's private quarters. Go there and find a body on the bed (you may have to fight a couple of Seducers along the way). This isn't Syl and you are informed of this shortly. You then leave the room and turn to your left. You can activate the statue to reveal a hidden path underground. Make your way through the tunnels killing enemies as you go; they should all be a breeze with the lower difficulty. Eventually you will get to an area where the marker turns green, go down into a circular room and kill the Seducers. The doorway out of this room is blocked and it took me an age to work out how to progress. On the left side, there is an urn with one of those green rectangular buttons that you need to activate. Doing this will open up a hole in the wall and you can continue along the path. Eventually you will get to Syl, kill her, take the heart from her corpse and exit the tunnels. Now you can head back to the Sacellum Arden-Sul and activate the heart at the altar in the middle. The ritual is completed and you talk with Sheogorath. A second later, the Duke of Thadon enters the room and a lengthy discussion takes place. Once this is over and you talk with Sheogorath again, the achievement will unlock.
