In order to get this achievement, you must play the story on Single player. Splitscreen or online will NOT unlock this achievement. Then you must complete the following levels to complete the achievement.
The Bridge
Spillways
Secret Labs
Countdown

Once you complete Countdown and if this has been done on realistic, during the credits both achievements will pop. This can be done on any difficulty but advised that if doing it on Realistic you can work towards your "Rainbow Six Legend" achievement.
