You first need to beat the story mode with the 16 default characters. One you do that you will unlock Helena as a playable character. Then beat the story mode again using Helena and then the achievement will pop after her ending movie and ending credits. You will also get an achievement for unlocking the Spartan character from Halo. Helena is the only unlockable character that you need to beat it with.
