Once you complete all cases, you'll probably have 12-24 hours (1-2 real hours) left till the helicopter arrives. While you wait you can level up by shooting the Special Forces guys in the Warehouse. At around 9am, you MUST make your way to the North Plaza to speak to Isabela (in Carlito's Hideout) or else you will NOT get Ending A and this achievement.
