Unlock the Jackanism chest on the Kraken level. Make sure you have 3 shrunken heads then activate Jackanism* near the chest.

*To activate Jackanism: hit RT,LT, and A at the same time.

The chest is in an open area with some tenticles, kill the tenticle on the left and there is the chest.
