In the first level with the chocolate when you get to the cake keep running into the chocolate "water" underneath. When the kid comes up saying "haha" with a post it note that means you have died. Do that ten times and this achievement is yours.
It takes awhile to die ten times in a row.... |

Just to add to the discussion:
If you are going for this it may take more than ten times to die it took me about 25 and the achievement popped. Also Nelson Muntz (the Ha Ha kid) will come up with a note somewhere between every 3 and 5 times of dying (at least if you are trying to get the achievement by falling into the chocolate). |
