The combo rating you seek is S+ and it isn't too difficult to attain. Around chapter 9 and 10 you'll encounter some heavily armoured dudes that take a fair amount of whipping to put down. Trap one with the bear trap magic, then quickly unleash the other three original magics you were given. You'll probably now be sitting at a B or A ranking. Now just start whammin' on the guy until S ranking comes up. Hopefully he's still stuck in the bear trap thingy. Now be careful. Hit him one whack at a time and you should get a quick time "B" button prompt. Nail it and you should get your S+. Worked for me. :)
