This is a storyline based achievement in Act 3-2 A dam fine mess.

Rescue Trishka from the hiding cell and watch the cutscene where you bargain to evacuate her safely, and she will get you off the world. There's about 10-15 enemies in the area outside the cell she's being kept in, once you kill them off the cutscene will start and the chieve will pop afterwards.
