I unlocked this after I beat the Rock Wraith and went back home to see my brother and mother. You CAN Reload, if your character or party member gets knocked out. I did it on normal.

If you are trying to get it for the first year make sure that you play through the deep roads. I thought at first that you would unlock it once going there, but you wont until after you travel through the deep roads.

Edit: A full year in Kirkwall is an act. If you complete an act without dying then you'll get the achievement. I reloaded every time my party member or myself went down.

One full year in Kirkwall is any ACT, the achievement is obtaibed when you complete an ACT without any character falling in battle. Any difficulty setting will work. A character is considered to have died when his/her health drops to zero and he/she falls unconscious.

Stock up on mages in your party that can heal.

Stock up on potions and preset your character tactics to use potions when health is low.

Constitution is you characters base the health and Cunning is their defense, for players that have The Black Emporium there are potions you can purchase to re-allocate all of your skill points and abilities, called Maker's Sigh then just get another one and do it again.

If a character does fall, then you can reload the game from your last save and still get the achievement.

If you decide to do the money XP glitch this will give you a mass amount of ability points (about 150) and enough defense for all of your players that it will be much harder to fell them in battle.

Lastly, there is a Life Ward Potion that, when a character's health drops to 0 instead of falling in battle he/she has health restored.
*Note I never got to try out the Life Ward potion if I can get an update if it does or does not work I will edit this solution, if you don't want to take the chance then don't use this potion until it is confirmed.

I got this achievement during Act 2 its the act where the qunari start wreaking havoc on the city. I used the mage class during this act and used the spirit healer's group heal ability before any of my party members got knocked out and before you fight the arishok make sure have Fenris as one of your party members because he'll persuade the arishok to fight you one on one but only if you agree and its a better advantage and you only have to worry about healing yourself but that wont be much of a problem if you use your dog to help you out. This is the method that worked for me I'm not saying its full proof but you can give it try.

The easiest place to obtain this achievement is Act 1. It's simple to get, and can be done somewhat easily on Normal, and definitely done on easy. Having either yourself as a mage (with at least heal and prefferably Spirit Healer as your Special Tree) or Anders in your party, you just play through the game as you usually would, and save often. If someone dies, just load the last save. Should be pretty easy with Mage or Warrior class, I did it with a rogue on Act One, the Deep Roads gave me a bit of trouble (because that Varric was locked in and I didn't have Anders, just a generous stack of Potions).

NOTES:
A) The only DLC I had when I got this achievement were The Exiled Prince and the Black Emporium, and this was based off of that time.
B) Add potions to your characters tactics so that you don't have to manually go over to the character, and have your Anders (if you are indeed using him) on healer or a custom set of tactics with lots of heals in it.
C) REMEMBER to always go to your last save the second a character dies, just one will throw the achievement off.
