Death Medals: Last Swarmite alive dies from ways below.
Progressive Death Medals: Kill enough to get bronze, silver, and gold medals.
Mega Death Medals: At least 10 of your remaining Swarmites die the same way.
Most of these will come in time, but if you still need some after the game is over here are some easy grind spots to help with medals;
Vaporized, Grind Spot: Beginning of level 4.
Abyss, Grind Spot: Beginning of level 1.
Electrocuted, Grind Spot: Beginning of level 9.
Burned, Grind Spot: Level 3, first checkpoint.
Impaled, Grind Spot: Level 11 when the boss is raining down arrows.

Dismembered, Grind Spot: Beginning of level 4.
Trapped, Grind Spot: Level 9, mid-level.
(Mega Death Help; Still level 9, just don't run all of your Swarmites at the same time. Get down to about 15 and try to kill them all at the same time with the traps.)
Asphyxiated, Grind Spot: Beginning of level 1.
(Mega Death Help; Level 2, When you have to sacrifice 10 guys in the holes, just keep going and run into the giant fog.)
Crushed, Grind Spot: Level 4, towards the end.
