Easy achievement. Just make sure you're playing in Arranged Single Play and NOT Story Mode. Beat the game on each difficulty that is offered to you:

No Problem
Heart Pounding
High Axiety

Achievement Unlocks once you beat the game on all 3 difficulties. Be sure to use a different Witch each time you beat the game to work towards "Cleared all characters"
