To obtain the cortex bomb you must purchase it under the operative tab in the abilities menu. You have to be RANK 4 in order to get this special grenade. The cortex bomb is, according to the game, a surgically implanted explosive that can be detonated when you are incapacitated. So get up close and personal to an enemy, get downed, and blow up the bomb to get your achievement.
To explode the bomb, press left on the thumb-pad/ analog stick when you are on the ground and hold LT until it explodes. Just make sure there are people around you when you do it.

| Adding to these solutions, a good way of getting the AI to stay on your body is simply by loading up a mission where you must carry a "datakey". When they down you, they will stay on the key to return it. Then, just go to the bomb and hold left stick and there you go. |

In order to get "a bit of a headache" you must detonate a Cortex Bomb and kill an enemy. The Cortex Bomb is an Operative weapon that can be unlocked once you reach Rank 4.
Once you add this weapon to your arsenal load up any mission you need to use a Hack Box on, Security Tower for example as the Renegades.
While hacking as an Operative let the opponent kill you then select Cortex Bomb on the left hand side and click down on the left stick until boom the achievement pops. |
