This is mostly speculation at this point, but I believe what the game refer to is weapon types. If you look closely at your weapons tab in the inventory, all weapons have their own little blurry circular images. A weapon called [Effect] + [Name/Type] + [Effect or Modifier] share the same weapon icon with [Effect] + [Name/Type] so a Fiery Dagger +1 is the same as a Deadly Dagger.

You do not have to have all weapons in your inventory at once. Picking them up or buying them one time during your games is enough. Thanks to NuffinButSteele that confirnmed this is tracked between all your characters.

I kept track of all the unique weapon names I could find through one playthrough, and the achievement popped on a Heavy War Pick during the end. Here they are:

Even though considered weapons by the Fighter and located in the wepaons tab at merchants, these do not affect the achievement. Thanks to MrBojeezy for confirming this.

Now, although these have unique icons it could be that as far as the achievement goes, it won't differentiate between a Short Sword and an Elven Short Sword, but I did keep a lookout for variations on the most common weapons but didn't find any. If you do happen to find a Elven Dagger or Dwarven Greataxe or similar (not on this list), please post a comment to verify that the Elven/Dwarven/Dragonborn/Ornate versions of a weapon doesn't count.

- The Tenderizer and The Gravedigger are 'special' weapons found during a story event, but they are purple in color and do not count.
- Some say the Tiefling Bastard and Bastard Sword doesn't count, while others unlock the achievement on these weapons.
