You can earn this achievement on Level 27

At the end of the level you will come to a large chamber. You will have to kill two of the worms you killed in level 7 (Queen of the Colon) to earn this achievement. The worms will come out of one of the holes and travel around the rocks towards the other hole. You must avoid the worms, and pull off all 4 eyes from both worms.
