You can earn this achievement on Level 7

At the end of the level you will come to a chamber with two rocks in the center, and 4 holes in the corners of the chamber. The worm will come out of one of the holes and travel towards another hole. You must pull off all 4 eyes from the worm to kill it and earn the achievement.
