Okay this is a very tough achievement if it's your first time playing through this game and will require a lot of patience. I managed to get it done in 3 hours and 49 minutes thanks to this fantastic video guide by TheMixedHerb on youtube.

He managed to beat the game in just over 3 hours.
These videos are so helpful and will easily beat any written guide. There are 12 parts of the guide but you only need to watch up to the end of Part 11 since Part 12 is just the ending cutscene. So below, I have listed parts from the videos which can be made easier for people who are playing for the first time by doing a few things different. Also, I will list some herbs which aren't included in the videos which will be a great help if you're like me and are always worried about health.
Firstly, there will be spoilers in this guide but you must have already played the game before, at least once. If not, then you won't be bothered anyway because you will have skip cutscenes to save time :D
So for the achievement you need to get 'A' rank. To achieve this you must follow these rules -
Beat the game in under 4 hours and 30 minutes
No first aid sprays (So don't bother picking one up if you spot one),
No saves (Avoid all saves points and Ink, which will take up space in your inventory)
No retries (Yup, you die then it's a restart)
Skip cutscenes (Very important rule!)
There is however one place where you can save the game which is halfway, the game will automatically ask you, so make sure you do! This will not count towards your final rank so don't panic. If you don't save halfway when it asks you then your asking for trouble.
There are a few story things you need to do for the A rank but the videos cover them so as long as you follow them you won't miss a thing.
If you get grabbed by a zombie or anything, rotate the left stick in a circle rapidly and keep pressing A as fast as you can to break free before the zombie or whatever can bite you. If you did it correctly, you will push them away before any blood is drawn. This may take a while to get used to and is pretty tricky to pull off. I ended up being a right muppet and pressing the buttons before I was even grabbed which then resulted in me getting grabbed straight afterwards so just don't go overboard.
Also, I don't know if pausing the game by pressing 'Back' acutally stops the time, I highly doubt it, but this is what I always did while following the video. If people message you then ignore them! You can't reply or you'll most likely miss the target time so make sure you remember this!
In case you don't know what herbs are and do, let me explain which each herb does:
Green - Regain some health
Red - Nothing unless combined with green
Blue - Cures poison
Red and Green - Full heal (Epic win)
There are a few others but this is all you really need to know. Combining green and green is pretty good too if your inventory is starting to get full but red and green is the way.

Okay, so below I will list parts in all 11 videos where I came up with some easier less risky ways to get past enemies & a few extra herbs, also some very helpful tips to deal with enemies or bosses.
I will list the time these occur in the videos in minutes and seconds. So get ready to start following the videos from Part 1 and check out these tips:
Part 1:
0:00 - In the cell you start inside is a green herb hidden in the corner.
1:50 - Shortly after walking through the door, you will notice the player (TheMixedHerb) walks near a wooden patio on the right. If you keep following the video, you will pass it. Instead, keep following the patio to the end then turn around, go up the steps, all the way to end to pick up a green herb. Then, carry on with the video.
2:38 - You visit this area a few times so make sure you watch the video carefully and watch the items the player puts inside so you don't set off the alarm. If you do by accident, then simply press the switch right next to items deposit to switch it off. Remember, nothing metal can pass through.
7:23 - Anytime in the game when you receive a file like this one, just simply press B to exit it.
8:32 - The player sets the alarm off on purpose so the zombies will not break through the window while doing the next part so don't worry.
11:15 - To push objects just simply walk into them, don't run. Once done, press X to climb on top.
12:51 - The computer code is NTC0394 in case the player typed it in too fast for you.
Part 2:
00:19 - I had no idea I had to save Steve fast in order to get an A rank. Ah well I managed to succeed everytime I restarted this. Once you get to the control panel simply choose 'C' then 'E' then enter it. In case your impatient like me and rapily press X to skip the words in order to press the buttons quick and you accidently hit 'A' then don't panic, just press it again to diselect it.
8:45 - I'm ashamed to say I got killed on this part. Yup, epic fail indeed. Basically you just need to do exactly what the player does in the video. However, I didn't know which way pointing the analog stick would take me towards the stairs and once I skipped the cutscene, I ran into the wall behind me then tried to make it towards the stairs but I got shot :( I don't know if one shot kills but my health was on yellow at the time and it did so just be very careful.
10:51 - First boss battle. This guy is a pain! You will come across loads later which you just run past but unfortunately you gotta kill this one and he does nothing but keep smacking you in the face. If you get unlucky like me and shooting in the corner like in the video doesn't work cause he keeps hitting you, just carry on or you'll just make it worst by moving. Be sure you check your health after every few hits to make sure he's not gonna take you down. Other than that, he's not hard, just annoying.
14:05 - You should see some ammo on the floor in the corner which the player doesn't get. Pick it up if you like. Never hurts to have some extra bullets :)
Part 3:
6:16 - Code for the lab room: 1126
9:15 - There are two herbs in view here but you won't be able to get them until much later on so don't waste time trying like I did.
Part 4:
4:05 - You can avoid getting grabbed like this by running right next to them. Although it may sound like a pretty stupid thing to do, it works, they lash out and miss you.
9:33 - There is ammo in this room that the player doesn't get, it's right at the back. Should come in handy.
Part 5:
1:09 - If this joker is blocking your path to the door right here like on the video then don't let him grab you, just blow him away with your pistol. It's not worth the risk of getting bitten. Make sure you reload your pistol afterwards.
2:42 - Same stuff here. Down that clown, don't let him grab you, then continue on.
7:49 - Major Alert! Do not do what the player in the video does and attempt to run through this crowd unless your feeling very brave I wouldn't recommend it at all. Instead, pick each zombie off one at a time with the pistol. Once it is safe, carry on following the video. I died here once cause I was constantly getting grabbed. It is so not worth the risk, it's better to just kill them.
9:08 - Be sure to kill this zombie. Do not bother getting grabbed, not only can you lose health but it just wastes time so kill him and carry on.
13:55 - Once again, make sure all the zombies are dead before coming back down. Running through a crowd is very risky.
Part 6:
11:36 - So this is the boss fight on the plane which is like a few seconds away from the save game you are allowed. I actually got killed by him my first time :( Do exactly as the player does in the video and you will succeed. I think I took way too long and he came up right next to me so be sure to follow the video to defeat him quickly. Shortly afterwards, the game will ask you to save. Make sure you do! As mentioned earlier, this does not count towards your A rank.
Part 7:
0:57 - You see that door in front of the player? Go through it. The player doesn't but trust me, go through. Once inside, on the left you will see an item box (which you store your items inside). Put everything in except your pistol, ammo and a green herb. Now continue on.
1:03 - Note here, if these moths grab onto you, they will put some sort of mini creature on your back if you don't shake them free quick enough. If they do, get ready to shake it free as it will start to bite you at any second. You will know when it is because Claire will stand still and look around. These are real easy to shake free compared to zombies. Also, check your status when passing through this area as you can get poisoned sometimes.
1:36 - In this B.O.W room, you should notice a blue and green herb on your way towards the bar code. Grab these if you like. The spiders might attack you and poison you, if that happens just use the blue herb. On all my runs I collected these, they don't matter much if you leave them so it's up to you.
3:15 - It may look like a waste of time but you MUST examine this pipe like the player does or you won't know the shape later. Trust me, I know :(
3:39 - Okay, this room is great. Remember I told you to put most of your items away? This is why. There is 5 herbs in this room. 5! Follow where the player goes and you can't miss them, two are on your right before reaching the corner along with pistol ammo, after hitting the lever then heading where the player does you will notice one herb on the ground right before the console that switches the power on and also, turn around and go to the other corner of the room where a dog is asleep on two herbs! If your inventory gets full, just combine green and green. It would be wise to kill all 3 dogs here as it doesn't take long and will stop you getting attacked every few seconds. Once you have the herbs, continue.
6:27 - Okay, now store all those herbs inside the box except keep two green herbs combined like in the video. Make sure your items remaining in your inventory are exactly the same as the video, so yes put your pistol inside, this is so Chris can have it later. If you don't, Chris is screwed. When the player moves towards the bookcase, you will notice a green herb on the ground. You can pick it up now or later when the player comes back. It makes no difference at all.
9:24 - There are two ammo cases in this room. One is to the left at exactly the video time (9:24) and the other is on a table next the herbs the player picks up in a few seconds.
12:35 - This is the worst boss fight in this game for me. So annoying. It isn't hard, it just requires good aiming with the sniper rifle and I mean good aiming. Take your time on this since you only have 7 shots. Do not let the enemy get too close to you or one of his tentacles may send Claire off the edge. As your zooming in with the rifle (hold LT), be sure you are aiming at the heart, do not miss! You may not do this on your first go. If you run out of ammo or die, hit 'Retry' so you can have another go. Once you beat him, quit the game and restart from your plane save. Why? Cause you need to be able to beat him at least once before doing the whole of Part 7 again, or if you don't you'll most likely just die everytime. If Claire gets poisoned, she can't be cured so don't panic right now. If she starts walking really slowly while you hold run, it means your health is on red so heal straight away with that green and green combination.
14:13 - Now your playing as Chris and are about to fight the worm creature you may have seen earlier as Claire. This guy is a piece of cake if you use the fire bolts in the bow gun. The player doesn't, he saves them for the final boss so it's your choice. I didn't have any fire rounds for the boss later but still managed to beat it without trouble so it's up to you. This method kills the worm in 5 shots when he's surfaced and not moving for a few seconds.
Part 8:
3:16 - Near the shotgun shells the player picks up is a herb. If your inventory is full up, enter the room the player passes next and shove some stuff inside the item box then come back and continue with the video.
4:46 - Seriously, knock this guy over, don't bother trying to activate the lift while he is there or he will grab you. Quick kill and your all set :)
7:37 - On the desk to the left of the cabinet you set the temperature on is two herbs. One blue, one red. Forget the blue and remember the red cause your inventory is about to get full up. You'll come back here later so don't panic.
8:39 - Hunters, these guys are tough! Do exactly as the video does and you should be good to go. If you take a lot of hits though, keep checking your health! You'll be surprised how easily these can kill you.
9:32 - The player picks up a green herb here, if your inventory is full then don't bother. You return here later so then you can grab the green and the red I mentioned earlier.
10:44 - Right, take this guy out. He is in your way and will only grab you so just fire with at him with your pistol until he won't get back up.
11:48 - Stop! Right here. There is a security scanner checking the small hallway your in for intruders. If you walk into it, a hunter will drop from the roof. You do NOT want that! Watch the pattern and when it's safe, run into the next area. Take your time!
13:47 - Here is another scanner. This one is more difficult to evade and I honestly don't know how as both times I managed to evade it, it was only because it switched off as it was about to see me so be extra careful on this one!
Part 9:
0:39 - Right at the bottom of the stairs, remember that green herb I told you to wait until later to get? Well, you should have enough space in your inventory now so grab it, and follow the video which will take you to where you killed the hunters with the shotgun earlier. Before using the lift however, enter the lab which you were in earlier to grab that red herb I mentioned! If like me, you pick up the blue instead by accident then just eat it or whatever cause your gonna need it right now.
1:44 - Okay, kill all three zombies here. It will help navigate the room now and later. It doesn't take long and you won't risk losing health so yep, go crazy. Well, make sure you don't waste all your ammo of course.
2:45 - This is the only time I will mention this but make sure you let the video play into each room before you enter it on your game from now on because some hunters are camping behind doors now so you wanna make sure they don't get the drop on you.
3:11 - Grab the pistol ammo to the right on the shelf.
4:37 - DO NOT ENTER THE DOORS AHEAD WITHOUT FULL HEALTH! Seriously, make sure you have green status or you may get killed. There are two hunters behind this door and you just need to run past them, it is not however that simple as they will try there best to stop you. I died here once as well because I had low health so be very very careful.
8:50 - Well, here is another part I died. A hunter drops down from somewhere and on the video the player has no problem killing it with the pistol. Unfortunately for me, the hunter kept on clawing me and wouldn't fall over and I died before I had chance to heal. He almost took me down on my second go as well because the same thing happened but this time I kept my health stocked up! So just keep checking your health.
Part 10:
1:08 - Take your time before rushing inside the water here to grab the object, wait until the creature moves away. I only got electrocuted once and I was fine, it isn't hard at all just rush in and out like on the video just make sure you enter the water at the right time.
12:23 - There are a few herbs up ahead on the ground but there are a lot of zombies around you so be very careful picking them up and remember to keep checking your health!
Part 11:
4:51 - Giant Spider boss, which you don't even have to fight as shown on the video. If you wish to avoid getting poisoned, after picking up the object near the body, give the spider a few seconds to look your way then start heading back.
9:02 - Okay so you save Claire now but if like me, she got poisoned during the boss fight on Part 7 then Chris will need to find a serum to cure her. The player didn't get her poisoned however and does not visit the place where the serum is so I will tell you. Backtrack to the elevator, go down, follow the corridor and enter the first double door room you come to (it's just before the corner which takes you to the room where the giant spider is). In case your not sure then go back in the video to 3:11 and you will know it. Once inside, there is a lift almost in front of you (not the one you used last time). It is only a few steps away. Use it and it will take you down to a room with 3 zombies in. Kill them, then to your right is a shelf with the serum on. Collect it and return to Claire. This won't waste a lot of time.
9:25 - You are now playing as Claire and if earlier she was poisoned, her health will be on danger so heal her right away. Now, before leaving this room be sure to have two (I had three which I recommend) super herbs. In other words, herbs that are combined. I had one green and red combined and two green and green combined. You make sure you have these or you will die on the next part. Claire won't need anything else except these herbs so be sure to get them out of the item box.
10:42 - Don't let this thing crush you or you will obviously die! Just follow the video and you should be fine :)
11:43 - This is the reason you need those super herbs. This boss guy will keep swinging his axe which will hit you twice as your running away. Watch the video first to see how the player does it. I messed up on this and got hit three times nearly because I had the analog stick the wrong way as soon as I skipped the cutscene. Remember, HEAL AFTER EVERY HIT YOU TAKE OR YOU WILL DIE!
18:06 - If running low on health, there are two herbs in the back of this room
21:28 - Before leaving this room, be sure you have everything. I had at least three combined herbs (all green and green) and everything the player took on the video. I was however missing the Bow Gun fire rounds cause I used it on the worm earlier so I had to just stick with regular rounds.
23:40 - Feel free to blow this zombie away with the shotgun instead of getting grabbed. You won't need the shotgun for anything else so you might aswell.
24:51 - As soon as you go through this door, you will see the final boss near Claire. As soon as you can control Chris, shoot the boss like mad before she can harm Claire.
25:21 - Well, final battle now. Use the same weapons as the player does on the boss except for the flaming bow gun rounds if you already used them earlier. If you did, I only used about 11 normal ones anyway out of the 60 I had, the rest I just used the grenade launcher. You will be attacked a lot by the boss and little creatures so keep checking your health. Once it's took enough damage, the boss will change into it's final form. Follow the video, pick up the gun on the wall behind you, now just aim it like the sniper rifle and fire at the boss. It takes one shot to kill her but it's pretty difficult since she keeps dodging. Her attacks are easy to dodge. Shoot, shoot, move around while she does her attack (which is only a fire trail) then repeat. Once you get her, the game is done!
After the credits or no credits if you skipped them, you will see your ranking. Congrats if you made it.
It will take a lot of patience and time and probably a lot of restarts before you get the hang of it but once you do, it's a nice challenge. Good luck :) I hope these tips helped you out!
