In Episode 4 Sceanario 10 "Fight to the Death" You must beat Yoder without making a mistake. This was by far the hardest achievement for me in the game. The long cut scene prior to the fight didn't help. The button presses go in the following sequence, figured this may help others:

RB to start fight
Conversation Wheel (I chose enough talk)
BXY
Conversation Wheel (I chose It's not about money anymore)
YXY and then immediately XBA
Cutscene, Yoder pulls out knife
RB X RB A RB
LB and RB together followed by X
Mash X to fill the circle
Mash Y to fill the circle.

Try to pick up the can...won't happen though, Once this is done, the achievement should pop. Just completed it myself!

*Update* Per Adjutant Spark if you fail after one of the mid fight checkpoints you can go to dashboard and retry it from the checkpoint. This DID NOT work for me, but doesn't mean it may not work at all. If anyone else can confirm that this works, let me know or comment on this solution. Thanks!
