You will encounter your first three standing stones shortly after escaping Helgen. These stones offer a blessing, of which you can only have one at any given time.
This may be a bug, but you are not required to find 13 unique standing stones. Simply revisiting the ones you've already found will add to the counter towards unlocking this achievement, this may or may not be limited to only 1 re-visit per stone, or it doesn't work at all for others.
Below is a list with all stones, and the general direction you need to head to find them. I could go into detail about the location, but it's alot easier to head into the direction described and keep an eye on your compass for the following icon at the top of your screen. The icon you're looking for is a pillar of some sort... a standing stone. :-p
- Thief Stone - Mage Stone - Warrior Stone
These 3 are found after escaping helgen, hard to miss.
- Lord Stone To the southwest of Dawnstar.
- Ritual Stone To the east of Whiterun.
- Atronach Stone Between Riften and Windhelm, in the marshlands. Might be tricky to find through pointers, use the method described above if you can't find it.
- Shadow Stone To the south of Riften.
-Lover Stone To the east of Markarth.
- Apprentice Stone To the south of Solitude.
- Steed Stone To the northwest of Solitude... will require some climbing.
- Lady Stone To the west of Riverwood.
-Tower Stone Walk a straight line from Winterhold to Dawnstar, it's just about in the middle.
-Serpent Stone To the east of Winterhold.

You can get the achievement by traveling back and forth between just a few of the standing stones.
I have only found 6 different ones but I've been to them each multiple times so my stats show I have more than 13 found so I got the achievement!! Traveling to the Guardian Stones counts as 3 stones discovered each time so I recommend going back and forth between them and another one you've found.
Some people have had problems with using the same exact stone over and over and it seems you have to wait some time before going back to that stone. Remember, I had 6 different stones and I'm not sure how ofter or in between I went to them, I just go the achievement on accident and realized this is how. By now I've been to them all countless times each and my stats say I've discovered over 100 stones.

In order to turn spazerMANIAc56's solution into something useful, I scanned the map:
The red circles are to ease the search for the small red crosses. If I missed any, please let me know and I'll add them.
Edit: After going for the achievement myself now (yes, I know), it appears as though not every cross points to a stone. Either that, or I am too stupid to find them (which is a very real possibility). That the in game map is 3D doesn't really help pinpoint the location, but overall the map proved useful. |

In the Dark Brotherhood Sanctuary, there is a book. If I remember correctly, it's entitled "The Explorer's Guide to Skyrim". When you open that book, the standing stone locations get marked on your map. |

This is the best map I've ever seen for a game. It works just like Bing/Google maps do. Just search for what you want. In this case "Standing Stone" This will show you the 11 locations, (11, because the guardian stones have 3 in one spot) the map is super detailed so you can zoom in a plot the best path to it. you can also zoom in to the stone you are headed for and click "reset search" in the top right corner of the page. this will allow you to see nearby locations that you can fast travel to if you have discovered them already.

As others have noted I can confirm that there is a slight glitch in your favor. it does appear to count multiple visits to the same stones as I wen't back and forth a few times while leveling up between a few of them and the achievement poped when I discovered my 8th standing stone and I have not seen the others yet.
Here are the names a descriptions of the stones, (taken directly from the wiki)
The Apprentice Stone Those under the sign of The Apprentice recover Magicka faster, but are more susceptible to Magicka damage. Activating the Apprentice Stone increases magicka regeneration by 100% and increases weakness to magic by 100.
The Atronach Stone Those under the sign of The Atronach absorb a portion of incoming spell damage and have a larger pool of Magicka, but recover it more slowly. Activating the Atronach Stone increases magicka by 50 points, decreases magicka regeneration by 50%, and adds 50% spell absorption.
The Guardian Stones The location which marks three Standing Stones located directly outside of Helgen and Riverwood. They are likely the first standing stones the player will encounter, since they are on the path taken while being escorted by Ralof or Hadvar to Riverwood. The stones are The Mage Stone, The Thief Stone, and The Warrior Stone. If selected, they allow the player to specialize in magic, stealth or combat skills respectively, by gaining 20% more experience in those skills. Archery is considered a stealth skill in this context.
The Mage Stone: Those under the sign of The Mage will learn all magic skills 20% faster. The Thief Stone: Those under the sign of The Thief will learn all stealth skills 20% faster. The Warrior Stone: Those under the sign of The Warrior will learn all combat skills 20% faster.
The Lady Stone It can be found in Lake Ilinalta, on an island directly north of Falkreath. Activating The Lady Stone will cause your Health and Stamina to each regenerate 25% faster. The treasure chest for Treasure Map X is located underwater just east and slightly north of here.
The Lord Stone Activating The Lord Stone will add 50 points to your armor rating and 25% resist magic.
The Lover Stone It makes all skills improve 15% faster. If you have The Lover Stone active, you cannot gain resting bonuses. This makes it a useful choice for users with lycanthropy, since werewolves cannot gain any benefit from sleeping to begin with.
The Ritual Stone Once a day, those under the sign of The Ritual can reanimate nearby corpses to fight for them. Activating The Ritual Stone grants you the power to raise corpses to fight for you for about one hour of in-game time. The power also allows you to raise corpses that can usually not be reanimated, such as mammoths or giants. The bodies do not turn to ash after the effect expires and there is no limit to the amount of corpses you can reanimate. A hostile necromancer is guarding the place and a circle of candles surrounds the stone.
The Serpent Stone While the stone is active, the player can use a ranged paralyzing poison on opponents.
The Shadow Stone Once a day, those under the sign of The Shadow can become invisible for an extended period.
The Steed Stone It increases carry weight by 100 and removes movement penalty of armor. It also makes worn armor weigh nothing, making heavy armor a viable option for many character types.
The Tower Stone The Tower Stone can be found about halfway between Dawnstar and Winterhold, on a high snow-covered cliff. Once a day, those under the sign of The Tower have the option to automatically open an Expert or lower lock. |
