First off, this is more of an observation really, rather than a guide, so apologies up front, but this may help someone.

Although the description states 25 secrets in a single playthrough, I think this is misleading. I played the game as the Mage first of all and found approximately 10-15 secrets. I then restarted the game on Heroic difficulty continuing on with my levelled up specs, but this time as a Ranger and after finding roughly another 10-15 secrets the achievement popped. So, I think that provided you don't restart from level 1, your secrets carry over to the next runthrough.

Bear in mind that most secrets can only be found by a particular class. So, Champion secrets won't be visible to Hunters etc. The Champion's secrets are easy to find because they are big areas of wall that have glowing yellow cracks. The Ranger sometimes has blue footprints leading to his, and are small blue glyph on the wall. They can sometimes be hard to spot. I found the Lore-master's one's hardest to find as she only has a small glyph with no extra assists like footprints.

I also believe the the quest "Cult of the Lidless Eye" provides 7 secrets, which are scrolls that you find in the latter part of the game after beating certain enemies etc.

All of this may be different in multiplayer, because if you have 2 people playing as different classes, all secrets found may count for both players. But that's just my theory :)

I found this from a user on the war in the north forums
"Hmmm. this is from memory so if I missed one please let me know.
She has 2 in Fornost, 1 in The Barrow-Downs, 1 in the Entmoors, I haven't found any for her in Mirkwood yet. I haven't found any rooms for the Elf @ Mnt. Gundibad, Urgost Lair or Cairn Dun.

The Dwarf has 3 in Fornost, 1 in the Barrows, 1 in Mirkwood( the ranger has a hidden stash in this room so you need a partner to get all the secrets on this level,) & 1 in the Entmoors, 1 in Mnt. Gundibad( needed for Mithril quest,) there are no secret rooms at Urgost Lari or Cairn Dun."
