This is an unmissable storyline achievement. You will receive this after defending the interior of Mt. Gundabad against a number of waves of orcs.

The mounted crossbows on either side of the chamber are the most effective approach, although they don't cover the rear of the chamber, so if any orcs slip through you may need to switch to ranged attacks or melee them. If you are playing solo without co-op partners, note that the AI will not use the mounted crossbows.
