I got this by playing on Novice.

Select the bottom ship of the 3 that appear in the middle of the screen when choosing to play as 1 or 2 players.

Doesn't matter if you die, or how many continues you use, just get to the end of the game and the achievement will pop when the Mission Complete screen has appeared.
