To get this achievement when selecting new game you must select the Tanker - Plant.

Once you beat the game and after the credits roll you achievement should pop once you get your Rank for how did in your play through.

I started off by just doing a few shots at tanker to get the dust off seeing I have not played Metal Gear since 3 came out. Then I did the Plant and it did not pop. I also confirmed before my "Full" play through that i had completed the Tanker and plant on the same difficulty.
