I got this on the first panel in Level 1 where Iron Man joins both Wolverine & Hulk. After you defeat the enemies, there will be a few items which you can smash (BEFORE going to the target circle to continue the level), which will give you ink and health. Make sure you pick it all up, then move to the continuing circle. Achievement unlocked!

Using "Hero Up" (press the circle between your three characters), this is easy and can be done on nearly any panel. Make sure the Hero Up meter is full and that you have full ink towards the end of the panel. Use Hero Up (which will restore everyone's life to full) and walk through the exit. Whoomp, there it is.
