Completing the game on the hard difficulty can be quite challenging, because of chapters 5 and 6 in particular. It can be terribly confusing as the game gives you no clue as to what you need to do to complete the levels. This will result in countless, aggravating trial-and-error deaths as there is only one way forward and many, many ways to fail.

Because there is no Ch. 6 completion achievement, here is a video showing how to beat the final boss in Ch. 6. He's not hard, but he is tedious. Generally what you want to do is lure him into those electric mine traps (the ones with the red rings that shock you if you move too fast) as shown in the video, when he tries to attack you he will be shocked, and this will be your chance to attack. Just rinse and repeat and he will go down eventually.

Regarding the difficulty achievements, they DO stack, so you should play the game right from the beginning on the hard difficulty. The only hard part for most of the game is figuring out where to go and what to do in order to progress forward, which is unaffected by the difficulty setting. Again, the only time where you'll notice the hard difficulty kick in is in Ch. 5 and 6 where you are required to fight off a lot of enemies.
