From the "Astral Home" screen, click the start button, which will take you to the "Edit License" menu screen. You have four choices: Player Card, Title, Country/Region, and Map.

I chose the "Title" option by clicking "A" and then choosing a title. Once you have made a choice, click "A" to ok your selection.

When you return to the "Astral Home" screen, you should hear the beautiful bleep of an achievement - A Soul Coalesces.
