It would be impressive to avoid this achievement while playing the game. Spamming the primary attack button (X), or even your secondary attack button if you have a melee weapon in that slot, will typically register an attack chain. You can't be hit in the midst of the combo for it to count.

Your special combos that you unlock as you advance count as well, but you'll get this with no problem long before you get very far in the story.
