Just wanted to point out in addition to the solution above, for me (a little bit better than an average gamer) the hardest part was the end of the Diamond Mine level where you have to defend your position until the helicopter arrives. An easy way to stay alive was lay down on top of the center building facing the stairs, killing everyone coming up. You will have to revive your partner a couple of times but you`ll be fine!!

OldSchoolBloodI did not see this posted anywhere, but to unlock Extreme you must beat the game once. Completing the game on "Easy", will unlock extreme.

jjaldanaIf you save right before the end of a level, you can load up the save for a friend and they will get credit for beating the level on expert. You could probably trade with people between the 3 versions of the game.

Oddly enough, this is easier in single player, since you get more checkpoints, closer together. Personally, I did the whole game split-screen co-op on medium, and did the game on extreme solo, and my solo playthrough was easier.

Knowing the whole game helps a lot. Just take your time, use the sniper to kill enemies from afar (like in the snow level) and the machine gunner anywhere else.

As Mtld mentioned before, playing solo works best.
And knowing the game will do the trick as you know what to expect. Some parts are harder for some, I had little or no trouble in the diamond mine, but the nuclear submarine map really took some effort. Find out what works best for you and after this playthrough you will know the game.
As for stating the obvious: use cover, reload in time, etc. like every shooter.
A few pointers, which worked out for me:

    Take your time,
use sniper and the masterkey shotgun frontmount, forget about machinegunning your way through. Use up all your grenades as supply crates provide new ones. Gas containers and other explosive items can signifficantly help you but can explode only once so timing is essential to make them as efficient as possible.

    As for your partner,
the a.i. machinegunner, keep him close and have him watch your back as some maps/levels have multiple paths where a single npc can take you out from behind.
Inevitably you or your partner will get downed, stay close.

    About the multiple routes,
make sure to clear them all before proceeding. Alot of enemy action is scripted, so they'll do pretty much the same every attempt. iow youll have to be in the right place at the right time and all clear when the next scripted event starts. Also backtracking and looking for another way in works well since the enemy's attention is still drawn to your first entry point.

    Save
as much as possible. For some parts I remember saving after every single kill just to keep going.

    Final fight with the bmp,
I got lucky (glitch?) when the bmp got stuck behind the chopper and me and my partner could fire upon it from the far right side (bunker with supply crate).

By far one of the hardest achievements I've ever obtained, good luck! and remember, your cousin dances naked...

Most players forget that you CAN SAVE ERVERY TIME on EVERY PLACE! The game don´t save only the checkpoints. The game take´s your "new" save as new checkpoint!

On solo it´s extremely easy. So simply save ervery time if you want. Go to -save game -make a new save - continue. Don´t overwhrite the campaign save!

Be careful! Don´t save in "action" like you are in the middel of a fight. I crashed a save on the tanker, so i couldn´t reload the "quick save" so i must reload the campaign save and i have to replay the entire mission.

This solution make´s the game very easy on every dificulty.
