To get this you will need to do 50 counter attacks.

To perform a counter attack, when an emeny is about to hit you, a button sequence will appear (usually button Y). When this comes up you have to press it to perform a counter. Do 50 of these and you'll get the achievement.

To gain more you can just stand around without hitting anyone and just wait to counter. You should get this on your 1st playthrough though, i got it i think towards chapter 3
