I obtained it after gathering all the fragments, finishing the seventh memory and then replaying the fifth memory. You'll notice upon loading the memory that a golden bridge will lead to the second door at the end of this level, so all you have to do is replay to that point and cross over, entering the previously unreachable gateway. Following the "scene" here, the achievement will pop as the credits begin.
According to J Pinder, all Fragments ARE a requirement for this achievement. Even though you can see the doorway upon replaying, the golden bridge won't appear until you've collected all the Decipher Fragments.
It's called "Breaking the Loop", not sure if that's needed for anything since this is still currently listed as Secret. |

In Order to "Break the Loop" you need to collect all the decipher fragments.
Once you have all of them. Complete mission 7, and you will be returned to mission 1. it will now be called, the end is only the beggining. Play up to mission 5, or go to the DNA screen and skip to it. Once you come to the end of mission 5, a golden bridge will let you cross to the exit door.
