Probably best to go for this as early as you can. Weaken monsters and save what little ammo exists by limiting your shots to when they're down or you estimate their health to be low.
If you find yourself at the end of the game and still haven't won this one, you should be able to get it at the boss. Collect the shotgun ammo, medkit, and fire axe from beside the first two spotlights (you don't need to disable the boss' life support). Walk over the proceeding bridge and wait on the walkway. Standing on the bridge will anger Mr. Veggie into throwing furniture at you, but if you stand right the cage on the bridge should shelter you and enemies won't pursue you if you need a breather. A gate further down this catwalk will open periodically and release either a siren, prisoner, or or knife-prisoner. **I was on normal
-Two swings with an axe and 1 shot should take care of a siren, sometimes 2 shots.
-3-4 swings on a prisoner and 1 shot
-3 shots for the knife-prisoner, it saves what little health you've saved.
Took a few tries but I had around 26 spare shots and 5 medkits I believe and barely made it with 1 shot left. It was my fault for not paying attention to the details on this one and trying to save ammo earlier in the game by beating downed enemies instead of shooting them like the achvmt specifies.
