This can be done quite easily playing single player in the Campaign.

At the very beginning of Chapter 3 you will be ambushed by a load of zombies in a very small room. Wait a few seconds and one or more of your squad members will become infected very quickly.

You will know they are infected as they will have a kind of Bio-hazard symbol next to their life bar. Once this happens, just blast away with headshots. Took a few shots before they went down in my game but once they do the achievement will pop.

Also, once you have this you can then revive the fallen squad member and carry on playing as normal with them still in your squad.
