To get the UFO ending you must first at least beat the game once on any difficulty with any ending. You must also complete the Born From A Wish sub-scenario to make the Blue Gem appear. Start up a new game and as soon as you get control of James, move towards the camera and pick up the Blue Gem at the back of the Bathroom.

Now proceed through the game normally and "use" the Blue Gem in the following areas:

1. The garden in Brookhaven Hospital after defeating Flesh Lips (Boss).

2. In front of the row boat on the dock after leaving the meat locker / historical society.

3. In room 312 of Lakeview Hotel BEFORE watching the video tape.

After that the ending will play! (Not as funny as the SH3 UFO ending unfortunately)
