In order to unlock the difficulty you must beat the game on easy, normal, and hard riddle difficulties first. You won't unlock it just from beating hard riddle difficulty. I can confirm this myself. Just to be clear, you have to complete the game 3 different times on 3 different riddle difficulties to unlock it. Make sure you "load" your save data before starting a new game to ensure that your new game is continuing from the right ending file.

Once you beat the difficulties, next time you select hard mode and save you will see you're on "extra" riddle difficulty.
