For this achievement you simply have to kill at least 75 enemies throughout the course of the game using melee weapons.

The achievement popped for me when I made it to the Brookhaven Hospital (Normal version) and killed about 5-6 nurses there. I had tried to kill as many enemies as possible since the start of the game. The beginning may be rough but once you get the Steel Pipe (Otherworld Shopping Mall) the rest is easy. Just make sure that you kick them when they're down to finish them off.

Also on a side note, playing on a higher action level increases the amount of monsters in the game. I did this on Easy though and still got it with an hour of gameplay to spare.
