This achievement is for getting 100 crystals cumulatively. This means you do NOT need to have 100 at a time, which is good since the game caps you at 99.

The best way to get crystals is to play a level with a 9-STAR or 10-STAR difficulty on CHAOS and use your True Triple Attack (used by pressing the Left Trigger and Right Trigger at the same time when your triple attack gauge is full) on a large group of enemies.

You will likely get this while going for the achievement for beating the game on Chaos if you use the True Triple Attack as a part of your normal play style.
