For those people, who like me always make the wrong turn, no matter how well a guide is written, I've found a video on Youtube.
Act 2; Iorveth's Path.
Mission: Where is Triss Merigold.
After you've gone through the mist for the 2nd time and have met Vernon Roche you get two options to get to Triss. To get this achievement you have to follow the righthand marker on the map. Go up and after defeating some of those exploding monsters you can drop down and climb up to reach the beginning point of the video.
The achievement should pop at the six minutes mark.
