Here's the video that I used to get this achievement done. Skip ahead to the :47 mark to skip the lucky shot the guy shows.
Also of note. The guy that he shows first never had a frag on his belt for me. There are a total of 4 guys on ground level that you can see from your insertion point. The one to your left has a frag but is too close. The other two guys are in the middle and one of the two should have it attached to the belt on his back (right side). The fourth guy is back near a explosive tank.
Figure out which guy has the grenade and wait for him to turn and walk away from you during his patrol. It may take a few tries, but luckily you can just restart if you miss your shot.
NOTE: Not my video, but one of the better ones I found.
