Here's the video that I used to get this achievement done. Skip ahead to the :47 mark to skip the lucky shot the guy shows.
Also of note. The guy that he shows first never had a frag on his belt for me. There are a total of 4 guys on ground level that you can see from your insertion point. The one to your left has a frag but is too close. The other two guys are in the middle and one of the two should have it attached to the belt on his back (right side). The fourth guy is back near a explosive tank.
Figure out which guy has the grenade and wait for him to turn and walk away from you during his patrol. It may take a few tries, but luckily you can just restart if you miss your shot.
NOTE: Not my video, but one of the better ones I found.

| This is easier than it looks and sounds :D you can shoot the grenade from dead soldiers :D I did this on the second mission, one of the two reinforcements has a grenade on his belt, kill him then shot the grenade. hope this helps. |

I tried for a while before I got this but I finally managed it on the fourth level, Opernplatz, a few minutes after getting the following achievement:
This is a bit of luck as most of the time you don't have time to check if a soldier has a grenade. I would suggest going into focus mode (RB) and then aiming around the waist. If he has a grenade great, if not keep on trying.

| This is actually a really really easy achievement to get. Play kill tally and wait for someone to show up with a panzerfaust in the distance shoot the missile on it and you should pop the achievement. |

The best place to do this achievement is on the:-
KAISER-FRIEDRICH MUSEUM,
About 5 or 6 minutes in after you pick up the Mosin-Nagant from an upstairs room, head back down stairs to the street, as soon as your on the street look RIGHT & at the far end of the map there appears to be 5 german troops standing to attention, all you have to do is shoot the grenade off the soilder its easy as they are not moving and the grenade is facing you. if you do it correctly you should cause a chain reaction and get 5 kills in 1.
which means you will get the following achievements
COOKING OFF (20 gamerscore) &
DOUBLE DOSE (kill 2 enemys with 1 shot) also worth 20.

| I found this easy to unlock playing the single player horde mode, the first few waves had Germans with grenades on their belts, and they start about 130metres away.. Play on lowest difficulty for pin point shots |
