This achievement is called "Dragonheart" and Santonium's solution is only slightly correct. The definition is indeed to spare or save Saskia, but not when she is sick. You have to do that for the main plot anyway. This is after the fight with the dragon at the end of the game. If you chose Roche's path, simply don't kill the dragon and walk back to Loc Muinne. If you chose Iorveth's path, you'll automatically use the dagger and save her, unlocking the achievement. That being said, if you want the "Being Witcher George" achievement for killing the dragon, you must choose Roche's path, as getting the dagger to save Saskia is part of the main Iorveth plotline.
