Once you have earned a total of 135 medals in single player, you will have unlocked the Trials Trophy. After doing this you will unlock the Flatliner event which consists of the game's Extreme tracks.

The easiest Extreme track is definitely the first one; The Truth is Out There. Select it and try your best to finish it with the skills you've learned throughout the game, especially those learned to acquire your A grade license.
