Load up underwater slaughter in act 1.At the begining of the 2nd part of underwater slaughter shoot the pipe on your left and the AD-4 will appear.Shoot it till it falls and blows up.Reload the checkpoint and rinse and repeat 4 more times and the ach.will pop.Thanks DeathIsland for pointing out it is the 2nd part.
