Achievement is tied to the normal progression of the campaign.
Mission 12: Shattered Mountain - Max score of 225.
Weapon Challenge:
Personal Defense - Kill five enemies consecutively with a PDR while remaining undetected. Challenge is very simple and can be accomplished in the early stages of the mission. Completing this challenge will unlock the SA58 OSW Rifle.
Tactical Challenges:
Swamp Fox - While playing on Elite, reach the swamp without beind noticed. You will end up eliminating 5 of the HVTs before you reach this point. If you follow what is in the video, you should have no problem completing this challenge.
Countdown - Kill all the HVTs at the farm in three seconds or less. It best that you remain stealthy to also earn the Swamp Fox challenge. It is best that you eliminate all the guards around the HVTs first using stealth kills and sync-shots. Once all the guards are eliminated, mark the three HVTs and the last guard for a sync-shot and let it rip.
HVT - Kill the two HVTs that are fleeing in less than six minutes. This challenge may seem rushed, but it really is not. You will end up killing Tereschenko first and will chase Petrakov to a railroad where he tries to get on a train. Make sure you keep an eye on Petrakov's distance from you. If he reaches over 250 meters, you will have to restart the chase. Completing all tactical challenges will unlock the Fixed Stock.
After you Petrakov is eliminated, you will earn the achievement 'No Loose Ends'.
