In order to unlock this achievement, you have to beat campaign mode in co-op. You are able to do the missions on any difficulty and any order using mission select, just as long as you have at least one other human partner with you throughout the entire campaign.
Please take note, this does not have to be done on one save. If say you have started the campaign late and only partially completed it, you can go back after you’ve completed the campaign and use mission select to mop up any of them you missed in co-op.
