This achievement can be a real pain at times, but stick with it. It's a good idea to play through the campaign as you normally would and mark off which missions you miss the 80% mark on. That way you won't need to manually check the briefing for every mission to see which ones you have left. (This is done by selecting and starting a mission and after the intro videos go into challenges and then statistics.)

There is no "magical solution" for getting max ghost score yet but I will do my best to outline the best possible way to get it.

Here are the criteria for ghost score (as stated by the game after each mission):
Stealth Combat:
> Silenced Kills
> CQB Kills
> Sync Shot Kills
Precision:
> Total Accuracy
> Headshots
> Civilian Casualties

The basic premise behind ghost score is to maximise all of the above except civilian casualites which should be as low as possible. The only one that doesn't seem to make any impact whatsoever is accuracy.

The 'trick' to most (if not all) missions is to stealthily kill as many enemies as possible. Try to always use your mark & sync (RB) and go for headshots and close quarter stealth kills (X). The key to high ghost score lies in abusing those 3 things.

A few very important pointers:
-Me and my friends did the mission Gallant Thief and didn't kill a single enemy or get detected until the unavoidable action scene at the end. We got a ghost score of 57. In effect you get NO ghost score for enemies you do not kill. This means that you can and should sync kill (RB) and headshot as many enemies as possible, preferrably without detection.

-Use silencers on your weapons to avoid detection. A good baseline is to have a sniper and a PDR or SMG with you. Using armour-piercing ammunition can make it easier to get detected since bullets travel through walls and possibly past other guards who will notice it.

-Deaths/Restarts have no impact on ghost score! We played a mission for hours and died/restarted possibly hundreds of times and still got 100% ghost score.

-Civilians killed by the ghosts, including those under AI control, count against your ghost score, if you see it happen, restart checkpoint. Enemies killing civilians has no negative impact.

-Challenges do NOT affect your ghost score, it's up to you if you want to complete them or not.

-Playing on your own can make it easier since you have complete control over everything that happens.

-Elite difficulty gives you the highest multiplier and so the highest ghost score

-The achievement states that you need "above 80%" (meaning 81% or more) but according to several commenters the achievement unlocks even if you only get 80%.

I got this achievement very simple by doing a lot of headshots on Veterean difficulty in Single Player.
Accuracy doesnt seem to influence the Ghost Score so just go through the levels (I prefered the TAR Assault Rifle) with 4x Scope, Target Laser, Silencer.
Getting through missions without beeing detected also doesnt seem to lower the score so you can do some syncronised shots then rush in with guns blazin.
When a mission constists mostly of "No Alert" parts then you have to work your way through it by picking one Target at the time and with the help of your team of course.
Most important if you have an wide open space area kill those targets first that are in high positions, remember the patrol routes and be patient.
After the first time playing the campaign on Veteran I just had three missions under 80%.
-The Mission with the Warhound (because I only used the robot and didn´t get any headshots)
-Norway Arctic (because I sneaked through the entire starting area and didn`t kill anybody)
-Nigeria (again way to few headshots)
