You are going to want to make a save right before you enter the final destination of chapter 13 in Riverspring Castle.

Upon entrance to that room there will be dialogue for Mors, Alester, and Varys (The eunuch) prompting for you to choose between Mors destiny and Alester's destiny.

For this achievement you want to choose Alester's destiny and this achievement will pop at the same time as the completion of chapter 15 achievement.

To do both you should make a save right before as I mentioned and pick the opposite one the second time.
