427,469 is the highest possible score. In order to achieve it, you must reach the final level (the Dragon's Lair) without dying, then you must play through this final sequence and die right before the final move four times, only completing the final move on your final life.
Important: This MUST be done in Arcade mode on Easy difficulty.
You can save the game at any time, so feel free to save at the start of each level to ensure that you can get through to the next level without dying. If you die, quit the game, then choose "Play Game" and then "Resume Game" from the main menu. Be careful not to save at the end of a level as points are being awarded or you might not get the necessary points (save at the start of levels to be safe).
One part that is tricky is the falling circular platform level, which you'll play four times. Two times, you must exit on the third stop (going further will result in death) and two times, you must exit on the ninth stop. I don't believe you can be sure whether the final stop is the third or ninth without trial and error, so definitely save at the start of these levels and plan on exiting on stop nine. If you die after the third stop, then quit/resume and exit on the third stop.
Wen you reach the Dragon's Lair, you MUST have exactly 288,494 points (which becomes 288,543 immediately after the level starts). Save your game here. If you don't have this exact point total, you missed something and will have to play through the entire game again.
Correctly play through the Dragon's Lair up to the next-to-last move (the 10th move in the sequence), which is an "Up." The final move, which is "Sword" (thrown at the Dragon to kill him), must not be executed (so you will die). When the level reloads, save. Repeat this process until your final life, at which point you will play the sequence in its entirety, killing the dragon, completing the game, ending up with exactly 427,469, and earning the achievement.
