Spoiler free version: In the postgame you will have a quest with a woman named Quince who can be found by going through the Pawn Guild and down the passage left of the Rift Stone. Complete this quest for the achievement.
---SPOILERS AHEAD---
Same as the first paragraph but better explained. In the postgame you will have access to the everfall, see here if you don't have access to it yet:
Grab a ledge in the Everfall by pressing RT next to or above a ledge (mashing RT helps), go through the large doors to named chambers which will contain new monsters or altered versions of ones you will have already faced. These will drop wakestone/wakestone shards as loot. You need to give 20 wakestones to Quince who can be found on the highest ledge in the Everfall (if you fall through the bottom you come out of the sky to fall through the Everfall again in an endless loop until you grab a ledge), also found through the pawn guild as mentioned above. Give Quince the 20 wakestones, a cutscene will then play which shows the wakes tones forming a portal below you in the Everfall, jump into the portal for this achievement.
---BIG SPOILER--- the Chamber of Lament houses the Ur Dragon which spawns 20 wakestones in the room you have to pass through to exit when you kill it. |
