This is for dying in the battle with the glowing guy after leaving the Everfall. After entering the Everfall, you get the quest to collect 20 wakestones. After you have collected them by whatever means, turn them in to complete the quest. Note that this will result in the permanent closure of the Everfall. After you trun them in, there will be a cutscene then you fight a glwoing, fellow arisen. Let him kill you. When he does, there will be a cutscene and soome credits, followed by a reload screen that will send you back to the beginning of the fight where you can proceed onwards as usual.

Leo AscendentYou can get it after, I killed myself when Salvan or whatever the Arisens name is, wanted me to kill him and take his place. So kill yourself before killing him, simple.

Easiest way I found to get this was to use the Godsbane blade on yourself while fighting the glowing guy during the first part of the Final Judgement battle. There is a separate achievement for using the Godsbane blade on yourself after completing all the Final Judgement quest by beating the glowing guy and taking over as seneschal.
