You need to clear all 15 battles without dying, your life does not regenerate and there is no magic. Thanks to the save game feature this one is not as hard as on the original Sega console. I would save my game at the start of every battle if I screwed up or just took a beating I would reload and try again. I got to the 15th battle with 5 bars of life left this way.
I used the dwarf as his jumping attack is great especially on the skeletons in the 14th battle.
